2026 Dock Diving Waiver & Liability Form

NOTICE OF RISK AND ACCEPTANCE

Participation in dog sports and water-based activities, including dock diving, swimming, and training, involves inherent and unavoidable risks. These risks include, but are not limited to, bodily injury, illness, property damage, drowning, dog bites, scratches, allergic reactions, and slips or falls. By entering the property or participating in any way—whether actively, as an observer, guest, or minor—you fully accept all associated risks and agree to the terms outlined in this release and the facility rules.

DEFINITION OF RELEASEES

For this agreement, the term “Releasees” refers to: Country Road Canines, LLC, Triple Play Dog Sports, and all associated individuals and entities, including but not limited to their owners, officers, employees, independent trainers, agents, volunteers, and property owners. This also includes any representatives or affiliates involved in the operations, maintenance, or facilitation of services on the property.

PARTICIPATION AND ACCESS TERMS

I understand and agree that Country Road Canines, LLC and Triple Play Dog Sports, along with their staff, owners, volunteers, independent trainers, and the property owner(s), retain the right to deny access to the property or services at any time, for any reason deemed necessary, including but not limited to:

  • Health concerns (dog or human)

  • Safety risks

  • Behavioral concerns

  • Non-compliance with rules or instructions

This right applies to participants, handlers, guests, and spectators alike. Access to restricted zones—including the dock, pool, training field, and staff-only areas—is prohibited without the approval of the property owner. Entry onto the property implies complete agreement to this waiver and all facility policies.

MINOR PARTICIPATION POLICY

Children under the age of 5 are not permitted on the dock or in the pool area under any circumstances.

Children ages 5–14 must be accompanied by an adult at all times and are not permitted to be on the dock, ramp, or in the pool without an adult present.

Handlers under the age of 18 must have a parent or legal guardian present on the property and sign the required waiver and participation forms. Event staff or instructors may require an adult to assist or handle the dog if safety concerns arise.

Minor Participant (Under 18)

ORIENTATION REQUIREMENT

Before using the dock or pool, all handlers and dogs must complete a one-time Intro to Dock Diving Orientation to review facility rules, safety procedures, and expectations. Returning teams from the previous season do not need to repeat this session.

Intro to Dock – Beginner Session: For teams new to dock diving. Similar to a private lesson, with an instructor guiding you through the dock experience after reviewing rules and safety procedures.

Intro to Dock – Experienced Session: For teams experienced in dock diving but new to our facility. Similar to a private dock rental, beginning with a review of facility rules and policies before using the dock.

DOG BEHAVIOR, SAFETY, AND CONTROL

  • Dogs must be healthy, current on required vaccinations, and free from illness (e.g., coughing, vomiting, diarrhea, parasites).

  • Dogs must never be forced into the water—no tossing, dragging, or coercion is allowed.

  • Only floating toys are allowed in the pool.

  • Dogs must remain leashed until they are securely on the dock and then leashed again before exiting.

  • Retractable leashes are not permitted.

  • No feeding dogs within 3 hours before swim time.

  • Life vests are strongly recommended for dogs new to swimming or dock diving.

  • One dog and no more than two people are allowed on the dock at a time without the facility owner's permission.

  • Barking is allowed up to 5 minutes on the dock. Dogs that bark excessively while waiting must be crated securely inside a vehicle.

  • Dogs must wait in a secured vehicle when not actively using the pool. Leashing or crating dogs outside the vehicle is not allowed.

FACILITY ACCESS & SPECTATOR POLICY

Guests and spectators must stay in designated areas and follow staff instructions at all times.

Anyone entering the dock or pool area must have a signed waiver on file. Spectators and non-handlers are still considered participants for liability purposes and are covered under this waiver.

Unauthorized access to restricted areas—including the dock, pool, training field, or dog yards—is not permitted.

All individuals on the property do so at their own risk.

PAYMENT, CANCELLATION, AND LATE ARRIVAL POLICY

  • Payment is due in full at the time of booking.

  • A valid credit card must be kept on file for all sessions and for any incidental charges.

  • Cancellations made at least 48 hours before the scheduled session are eligible for a refund.

  • Cancellations with less than 48 hours’ notice are nonrefundable, but credit may be used to reschedule for a future session.

  • Arrivals more than 15 minutes late are considered a no-show and will result in forfeiture of the session without refund.

  • A $50 cleaning fee will be charged for any dog elimination (urine, feces, or vomit) in the dock or pool area.

VACCINATION AND HEALTH POLICY

For the safety and health of all dogs at our facility, proof of current Rabies and DHPP (Distemper, Hepatitis/Adenovirus, Parvovirus, Parainfluenza) vaccinations is required. Vaccination records must be submitted prior to your dog’s first visit. Dogs with expired or missing vaccine records will not be permitted to participate in services until documentation is provided.

Vaccination Record Submission

Proof of vaccination (with expiration dates visible) must be submitted before entering the property. Please upload clear images or PDF copies through this form. Dogs are not allowed on the premises without current records on file.

If you are unable to upload them here, please email them to info@countryroadcanines.com

MEDIA RELEASE

By signing this waiver, I agree to fully and forever release and hold harmless Country Road Canines, Triple Play Dog Sports, and all associated individuals, including owners, employees, agents, independent trainers, volunteers, and property owners, from all liability related to:

  • Injury to myself, my dog(s), my guests, or property

  • Death or illness resulting from participation

  • Damages caused by my dog(s)

  • All risks associated with being on the premises

SEVERABILITY AND GOVERNING LAW

I understand that this agreement, including all rules, waivers, and policies, is intended to be as broad and enforceable as permitted under California law.

If any term, condition, or portion of this agreement is found by a court to be invalid or unenforceable, the remainder shall continue in full legal force.

This agreement shall be governed by the laws of the State of California, and any legal dispute arising from participation or presence at the facility must be resolved in a California court with appropriate jurisdiction. I willingly submit to that jurisdiction.
